(3) For medical devices already listed as National Health Insurance (NHI) reimbursement items, the NHI code must be submitted (please clearly indicate the NHI code).(4) A photocopy of the Medical Device License issued by the Ministry of Health and Welfare (MOHW) or a photocopy of the relevant approval documents issued by the Food and Drug Administration (TFDA) of the MOHW for the subject matter of this project (including equipment). (If the specification or efficacy column states "refer to the instruction manual/package insert," the manual/insert must also be attached).(5) A photocopy of the Pharmaceutical Medical Device Distributor License or Medical Device Dealer License. (If the product is self-manufactured, a photocopy of the Pharmaceutical Medical Device Manufacturer License or Medical Device Manufacturer License may be submitted instead).Notes:If the subject matter of this project is not recognized as a medical device by the MOHW (i.e., no MOHW Medical Device License, relevant TFDA approval documents, Pharmaceutical Medical Device Distributor License, or Medical Device Dealer License is required), relevant supporting documents must be attached.If the item is labeled "For Research Use Only" or "Not for use in diagnostic procedures," the requirement for the MOHW Medical Device License, relevant TFDA approval documents, Pharmaceutical Medical Device Distributor License, or Medical Device Dealer License may be waived, provided that relevant supporting documents are submitted or the specifications are reviewed and approved by the procuring unit of the Entity.If the Ministry of Health and Welfare issues new laws and regulations, the latest laws and regulations shall prevail.
